Mill Creek Nursing & Rehabilitation ranked as the third largest nursing home in Crawford County during the first quarter of 2026, data from the Centers for Medicare and Medicaid Services (CMS) shows.
The facility offered 79 available beds, surpassing Crawford County’s average by 12 for the same time frame. On average, it cared for 73.9 residents throughout the quarter.
CMS awarded Mill Creek Nursing & Rehabilitation an overall rating of 4 out of 5, exceeding the 3.2 average score recorded by Ohio nursing homes.
That quarter, Unger Park Post Acute was noted as the largest nursing home in Crawford County, offering 86 beds. Altercare of Bucyrus Center Fo followed as second, with 80 beds available.
As of the end of the first quarter of 2026, Mill Creek Nursing & Rehabilitation reported no fines or penalties.

Estimates from the U.S. Census Bureau indicate the country’s senior population is increasing, as younger cohorts decline. Between 2020 and 2024, Americans age 65 and older grew by 13%, while those under age 18 decreased by 1.7%. This group has expanded significantly since 1920, rising from 4.9 million to over 61 million by 2024.
A 2021 study estimates roughly half of these seniors, or about 28 million people, could need long-term services and supports, with over one-third eventually requiring nursing home care.
“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance, an insurance education foundation. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
